The University of California, Irvine's new healthcare campus, UCI Health, officially opened in December 2025 with 144 hospital beds and no gas line — one of a very few major hospitals in the world that runs entirely on electricity. CO Architects, which designed it with design-build partner Hensel Phelps, claims it is the only hospital larger than 500,000 square feet to do it.

The all-electric ending was not the starting plan. The project began in 2020, when the COVID-19 pandemic was putting unusual scrutiny on how hospitals function, pushing the design toward flexibility and the ability to absorb an influx of highly contagious patients. Meanwhile the University of California system was pursuing carbon neutrality in its buildings by 2025, making electrification a parallel priority.

When the design was being finalized, the market could not yet supply a fully electric hospital: at the construction-permit stage the plan still included a gas-powered central heating and cooling plant and a gas line feeding the kitchens. As construction went deeper, all-electric air-source heat pumps, air chillers and cooking equipment started coming to market — and the design-build team and the university decided to go all in, swapping the gas systems out of the project.

Hospitals are among the most demanding energy loads there are — room-sized MRI machines, medicine refrigerators and commercial kitchens all run around the clock, as design principal Fabian Kremkus of CO Architects told Fast Company. Meeting that demand with electricity alone is what makes the completed campus unusual at its scale.

Hospitals run 24/7 with some of the heaviest, least flexible energy loads of any building type, so they default to gas for heating and kitchens.

The equipment timing worked in the project's favour: all-electric heat pumps, chillers and kitchen gear reached commercial maturity while the campus was still under construction.

The UC system's 2025 carbon-neutrality goal gave the university a standing reason to say yes when the all-electric option became feasible.

Infrastructure transitions stall when the equipment isn't on the market yet. Designing a pivot path — and switching mid-build as suppliers catch up — can still land a world-first.
