In May 2023, St. Louis-based BJC HealthCare and Kansas City-based Saint Luke's Health System signed a non-binding agreement to merge into a $10 billion health system serving Missouri, Kansas and southern Illinois — two 14-hospital systems that had already spent a decade together in the BJC Collaborative, a 2012 alliance for pooling resources on cost, quality and population health.

The deal closed at the start of January 2024, about seven months after the announcement. The combined organization kept both brands by region — BJC HealthCare in the east serving St. Louis and southern Illinois, Saint Luke's in the Kansas City region — and runs from two headquarters, with a combined workforce of 44,000 employees.

The merger rode a post-pandemic wave of cross-regional hospital deals, which let systems pursue growth and diversification without the antitrust scrutiny heaped on same-market combinations: Advocate Health had formed from a $27 billion cross-state merger months earlier, even as LCMC Health's attempt to buy three HCA-owned Tulane hospitals in New Orleans hit a regulatory wall.
